Are fog lamps on when the high-beam headlamps are on?
Don’t use high-beam headlights. They won’t shine through the fog but just reflect the light back in your eyes, making it worse for you and other drivers. Use low-beams. In really dense fog, use front fog lights in addition to your low-beams if you have them.

How do you install a bulb on a headlight?
What you need to do is get the bulb retainer off of a standard headlight assy. (Bulb 9006) unplug the ballast assy. from the wiring harness and plug the harness into the 9006 bulb and install the bulb into the bulb retainer you just installed in the headlight assy.
